What is Ketamine?

Intramuscular (IM) ketamine treatment is a form of interventional psychiatry where low doses of the anesthetic drug ketamine are injected directly into a major muscle—typically upper arm/shoulder—to treat severe, chronic mental health conditions that haven't responded to standard therapies.

While ketamine is widely used as an anesthetic in higher doses, its use in mental health is "off-label" (except for the nasal spray S-ketamine/Spravato). IM ketamine is often seen as a practical alternative to Intravenous (IV) ketamine, offering rapid absorption and therapeutic effects without requiring an active IV drip setup.

How does it work?

Ketamine’s mechanism of action is likely similar to Spravato/esketamine as the drugs are similar.  It differs fundamentally from traditional antidepressants (like SSRIs), which focus on neurotransmitters like serotonin. Ketamine works on the brain’s glutamatergic system.

  1. NMDA Receptor Blockade: It blocks N-methyl-D-aspartate (NMDA) receptors on certain brain cells.
  2. Glutamate Surge: This blockade triggers a complex cascade that results in a surge of glutamate, the brain's main excitatory neurotransmitter.
  3. Synaptogenesis: This surge helps repair and rebuild broken neural connections and encourages the growth of new ones, a process called synaptogenesis. It essentially "rewires" neural pathways, breaking patterns of negative thinking and restoring mood regulation.

 

 

Conditions Treated

 

IM ketamine is primarily used as a robust, rapid-acting treatment for Treatment-Resistant Depression (TRD) and more. TRD is defined as depression that has not improved significantly after trying at least two different classes of antidepressant medications at adequate doses.

It is also explored and sometimes used for:

  • Major Depressive Disorder (MDD) with acute suicidal ideation
  • Bipolar Depression (when standard treatments fail)
  • Post-Traumatic Stress Disorder (PTSD)
  • Suicidality

The Treatment Process

Our protocol will be as stress-free as possible.

  • Preparation - via Assessment - A medical professional will perform physical and psychiatric evaluations to ensure eligibility. You will need to have a designated driver, as you cannot drive for the rest of the day. Drug screening and fasting guidelines are in place.  
  • Simple Administration - After a brief check-in and vitals monitor, a licensed clinician administers a precise dose of Ketamine via a small needle into the muscle.
  • The Quiet Phase - You will relax in a comfortable, reclined chair in a private room. The medication typically begins to take effect within 5 minutes. You may experience a "dream-like" or dissociative state where the mind feels separated from the body—this is a normal part of the healing process.  After a period of time you will likely receive one additional injection.
  • Monitoring & Integration - Our doctors or nurse practitioner clinicians and staff monitor you throughout the session (typically 2.5 hours). As the effects gently taper off, you’ll have time to ground yourself in our quiet recovery space before being driven home by a companion.

What to Expect: The Treatment Process

A typical IM ketamine treatment session is a carefully monitored outpatient procedure.

1. The Injection and Session

  • Setting: Sessions take place in a private, quiet, and comfortable room within a clinic or hospital.
  • Injection: A healthcare provider injects a precisely measured low dose of ketamine into a large muscle. The effect is typically split into two smaller injections given about 10-15 minutes apart to manage the onset of dissociative effects.
  • Experience: The effects begin within minutes and usually peak around 30-40 minutes.
  • Common Sensations: Feeling detached from your body ("out-of-body experience"), dream-like or floating sensations, distorted sights or sounds, and feelings of elation.

2. Monitoring and Recovery

  • Vitals: Medical assistants continuously monitor vital signs, including blood pressure, oxygen levels, and pulse.
  • Observation: The active "journey" lasts about 60 minutes, followed by a recovery period of an additional 60-90 minutes where you rest while the medication's effects wear off. Total office time is usually around two and one-half hours.

3. The Treatment Course

  • The process is almost always given as a series rather than a single dose. A standard course is:

Acute Phase: Two treatments per week for three to four weeks (6-8 sessions total).

Maintenance Phase: Tapering frequency to once a week, then once every other week, eventually moving to "booster" doses as needed based on individual response.
